The goal of discharge planning is to help you do so safely, and as smoothly as possible. It’s a collaborative process between your care team and you and your family. As mentioned, it begins when you’re first admitted, and it involves assisting you and your family in obtaining the resources needed for you to be safe at home, which can include:

We follow up with you after your discharge.

Depending on the particulars of your health and the nature of your stay, Gulf Coast Health Care will follow up with you with phone calls after you’ve been discharged. Even after you’re no longer physically in our care, it’s still important to us for you to avoid future injury and hospital visits. We’ll keep in touch with you, answer any questions you may have, and make sure your return home is going well.

Also, upon discharge, we’ll ask you to complete a survey regarding your experience with us and your care at our center. The input of our patients and residents is essential feedback that helps us pursue continuous improvement, meet your needs, and continue to earn our reputation for excellence.
